I understand that my likeness may be edited, copied, exhibited, published, or distributed, and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my likeness.
I release, discharge, and hold harmless Amethyst Headspa, LLC and its affiliates, employees, officers, agents, or any person or corporation acting under its permission or authority from any claims or damages arising out of the use of my likeness, including but not limited to any claims for defamation, invasion of privacy, or infringement of moral rights.
I affirm that I am over the age of 18 and have the right to contract in my own name. If I am under the age of 18, I affirm that my parent or legal guardian has read and consented to this release on my behalf.
